The attached papers present a detailed analysis of the Trust’s response to the five reduced bwhnational reports which arose initially out of the Mid-Staffordshire Hospital review and the main themes from the Sir Robert Francis’ review published in February 2013.

Attached is a report detailing all of the recommendations from each of these national reports. Within this report we have:

• Judged whether or not the recommendation applies to the Trust
• Identified who the Lead Person is
• Identified the Lead Committee
• Summarised the actual current practice within the Trust
• Identified the action(s) required and the associated dates necessary to comply with the recommendations
• Provided a narrative and RAG rating of progress

While Birmingham Women’s NHS Foundation Trust has a strong track-record and reputation as a high performing trust, delivering quality healthcare through strong cultural values, there are clearly learning points and accompanying actions we need to take out of each of these important reports.
